Channel 4 is making a new consumer series and would like your opinion about the service you receive when you go shopping. Some customers have raised concerns about poor customer service and we want to know if you have experienced similar problems.

The survey contains multiple choice questions about various types of shop and should take 5 to 10 minutes to complete. Please note that the survey only relates to shopping IN-STORE, not online.
